Registered Nurse (RN) Skilled Nursing Facility
Location: 1601 Petersen Ave, San Jose, CA 95129
We are seeking a dedicated and compassionate Registered Nurse to join our team at a skilled nursing facility.
RESPONSIBILITIES:

Conduct thorough assessments of residents' health status and needs. -
Develop and implement individualized care plans based on assessments.

Administer medications and treatments as prescribed by physicians.

Monitor residents' progress and make necessary adjustments to care plans.

Provide education to residents and families about health conditions and care options.
-
Collaborate effectively with interdisciplinary teams to ensure optimal care.

Document all aspects of patient care accurately and timely in electronic records.
QUALIFICATIONS:
-
Fresh or newly licensed RNs are welcome -
Experienced RN is preferred

Experience in a skilled nursing facility/long-term care/senior care is a plus
-
A valid and active California license as a Registered Nurse is required

Must have an active BLS through the American Heart Association
